With the lives of millions of cancer patients at stake each year, our mission to save more lives with immunotherapy—the greatest advance in cancer treatment since the 1940s—becomes even more urgent. Fortunately, CRI scientists are making new discoveries each day about our immune system and how we can harness its power to defeat cancer for good.
Immunotherapy is now considered the fourth pillar of cancer treatment, complementing and sometimes replacing surgery, radiation, and chemotherapy as options for patients. Through its decades of commitment to advancing scientific research that has made effective immunotherapy possible, the Cancer Research Institute has given new hope to cancer patients around the world. Our work, however, has truly only just begun.
While immunotherapy has been proven effective in many different cancer types—revealing its potential as a universal answer to cancer—a significant but unacceptably low percentage of patients responds to treatment. Fewer still achieve lasting cures. However, the fact that many patients with advanced cancer have been cured with immunotherapy drives CRI’s scientists forward in their pursuit of discoveries that one day will lead to the end of cancer deaths.
We know that reaching this goal is not only attainable. It is also inevitable. How quickly we get there, however, depends on our will as a global community to prioritize reining in one of the leading and fastest-growing causes of death worldwide. Will alone is not enough to accomplish this Herculean task. It takes long-term commitment, vision, expertise, efficient management of resources, collaboration, and, of course, money to put will into action that produces meaningful results.
This is where the Cancer Research Institute has truly demonstrated its essential and unique role, not only as a pioneer in establishing the field of tumor immunology decades before any other nonprofit organization, but also as a force for scientific progress whose momentum and expert foresight continues to push the field ahead and into the future.
At our core, we understand that the only way to realize immunotherapy’s potential to treat and one day cure all cancers is through scientific research. Laboratory studies on the immune system and its interaction with cancer lay the foundation for clinical applications, while clinical studies grounded in scientific rationale produce invaluable data on human patient immune responses to immunotherapy, data that can be taken back into the laboratory for further study. This two-way dialogue between the laboratory and the clinic forms an iterative loop that ultimately will reveal—precise molecular and cellular level detail—why some patients respond to immunotherapy while others do not, and how we can intervene therapeutically to save more lives from cancer.
In fiscal year 2022 (July 1, 2021, to June 30, 2022), the Cancer Research Institute awarded $22.2 million in grants, fellowships, and other funding to support bold and innovative laboratory and clinical research, train the next generation of scientific leaders in the field, power open-access bioinformatics and cancer information resource hubs, and provide seed funding to drug incubators designed to bring the most promising new immunotherapies into clinical development.
We also continued to deliver high-quality, expert-vetted information about cancer immunotherapy and clinical trials to patients and caregivers. This includes our Cancer Immunotherapy and You webinar series, the CRI Virtual Immunotherapy Patient Summit and its Spanish-language counterpart, the Conferencia Virtual CRI de Inmunoterapia para Pacientes con Cáncer, which we piloted successfully in fiscal year 2022 as the first event of its kind to present this lifesaving information entirely in Spanish, and our cancer immunotherapy and clinical trial information pages on CRI’s website, provided in both English and Spanish.
For the tenth consecutive year, CRI received in 2022 the highest rating from Charity Navigator, an independent evaluator of charity transparency, financial health, and impact. This places CRI within the top four percent of all evaluated U.S. charities to receive this rare designation. These ratings demonstrate CRI’s commitment to stewarding our donors’ goodwill to achieve the greatest impact.
In 2022, we welcomed to the board two new members, Michael Crawford and William Goldberg, whose experience in academic medical research and the health care industry, respectively, are sure to serve CRI well. Jacques Nordeman, a CRI trustee since 1982 and a vice chairman of the board since 1992, stepped down from the board after forty years of service and dedication to CRI and is now a trustee emeritus During his tenure, he introduced CRI to many influential business leaders and philanthropists, whose generosity enabled CRI to grow in new directions, expanding and creating new research programs that moved the entire field forward. We were also saddened by the passing of Julian H. Robertson Jr., who served for many years as a CRI trustee and trustee emeritus. His generous support allowed CRI to embark on bold research initiatives, most recently the CRI Lloyd J. Old STAR program that provides sustained funding to today’s cancer immunotherapy visionaries.
We could not put our mission into action without the support of our donors, community fundraisers, and others who rally behind CRI and our mission to save more lives with immunotherapy. As we approach the 70th anniversary of CRI’s founding, we look ahead to a better future for cancer patients everywhere—a future where cancer is no longer something to fear but instead something that can be managed or even cured with immunotherapy.

Overcoming T Cell Exhaustion and Improving Responses to Immunotherapy
Our immune system has evolved to protect us against harmful viruses and bacteria as well as our own damaged cells and cells that become cancerous. When the immune system’s response to these threats is swift and thorough, the danger is eliminated, and the immune system returns to a normal state of surveillance. During protracted immunological battles within our bodies, however, such as in chronic viral infection and the gradual development of mutated cancerous cells into clinical disease, the immune system’s primary cellular strike force, our T cells, can become “exhausted” to the point of no longer being able to effectively respond.
Immunotherapies today primarily rely on T cells to achieve results in patient treatment. Understanding how T cells become exhausted and developing ways to reinvigorate them or prevent them from becoming exhausted in the first place is a central question that CRI scientists are determined to answer. Their hope is that these answers will lead to new therapeutic strategies that improve overall patient responses to immune checkpoint blockade and other T cell-based immunotherapy approaches.
New Strategies to Reverse and Prevent T Cell Exhaustion
Mohamed S. Abdel-Hakeem, Ph.D., a CRI postdoctoral fellow at the University of Pennsylvania, made an important discovery about the impact exhaustion has on a T cell’s ability to recover and how this impedes their ability to “remember” and respond to danger should the threats, such as viruses, bacteria, or cancer arise again. He found that, even when exhausted T cells clear these threats, they can sometimes only partially recover. Notably, he identified specific gaps in cellular circuitry involved in switching on the genes within these T cells that would normally convert them into fully functional memory T cells. Understanding how these gaps, or epigenetic “scars,” develop may lead to therapeutic interventions to overcome this dysfunction.
Discovering a potential approach to prevent T cell exhaustion, CRI-Donald J. Gogel Fellow Hyungseok Seo, Ph.D., at the La Jolla Institute for Immunology, identified and characterized how BATF and IRF4, two types of regulatory proteins found within T cells, cooperate to counter T cell exhaustion in mouse tumor models. Dr. Seo and colleagues showed that overexpression of BATF in engineered T cells known as chimeric antigen receptor (CAR) T cells increased the survival and expansion of CAR T cells capable of infiltrating tumors. BATF overexpression is also correlated with an increase of molecules that promote anti-tumor immunity along with a reduction in immune-suppressing molecules. Overexpression of BATF also contributes to the generation of long-lived memory T cells that control tumor recurrence. These responses were dependent on the interaction of BATF and IRF4. Designing CAR T cells to overexpress BATF may prove beneficial to improving both immediate responses and long-term protection in patients.
Unmasking Triple-Negative Breast Cancer
An effective T cell-mediated immune response requires T cells to locate cells that carry specific targets, or antigens, that activate the T cell’s cancer-killing power. Other molecular signals that act as fail safes must also be engaged to initiate cell killing. If a cancer cell does not express the target, T cells will not recognize it as a threat and will move on. Cancer cells that escape the immune system in this way often can do so by losing these markers as tumors evolve. By masking their identities, these cancer cells go on to develop into tumors and can progress to disease.
CRI Clinic and Laboratory Integration Program Investigator Toni Celià-Terrassa, Ph.D., at the Hospital del Mar Medical Research Institute in Spain discovered a way to enhance the responses to treatment with checkpoint blockade among women with triple-negative breast cancer (TNBC). TNBC lacks three specific characteristics required for treatment with most available breast cancer therapies and is therefore considered especially hard to treat. When TNBC also lacks targets that the immune system “sees,” patients typically respond poorly to checkpoint blockade treatment. Dr. Celià-Terrassa, however, identified a specific transcription factor called ligand-dependent corepressor, or LCOR, that forces TNBC cells to express their abnormal antigens, making them visible to the immune system. His promising approach has the potential to benefit patients with TNBC and other hard-to-treat cancers that escape the immune system.
Augmenting T Cell "Super Assassins"
With the greater understanding of the mechanisms of immune suppression these and other CRI-funded studies have provided, CRI scientists are exploring how T cells can be modified to break through the hostile tumor microenvironment and carry out their cancer-killing objectives.
Yvonne Chen, Ph.D., a CRI Lloyd J. Old STAR at the University of California, San Francisco, created an enhanced type of chimeric antigen receptor (CAR) T cell to do just that. She engineered these super-assassin “living drugs” to produce their own immune-stimulating cytokines as they simultaneously seek out and eliminate cancer cells.
These molecules signal danger to other immune cells and trigger a cascade of events that recruits a patient’s naturally occurring immune cells to the tumor site to join in the attack. Dr. Chen’s approach eliminated both melanoma and brain cancer in mice and may be an effective strategy in treating human patients.
Dr. Chen also designed CAR T cells that moderate tumor blood vessel growth and improve immune cell infiltration into the tumor microenvironment. She also revealed how the structure of the CAR receptor determines persistence of these cells and, by fine-tuning this receptor and its signaling, generated the first CAR T cells known to outperform the gold standard of CD19 CAR T cells in preclinical models.
Diabetes Research Yields a New Strategy to Overcome T Cell Exhaustion
One of the most important aspects of basic research is its capacity for open-ended inquisitiveness. While the importance or applicability of new discoveries is not always readily understood, open-ended exploration grounded in scientific rigor and discipline ultimately expands humanity’s knowledge beyond what is already known—providing a framework for fundamental discovery that transforms the world.
History is full of anecdotes about accidental or serendipitous discovery, where scientists focused on solving one problem discover solutions to another. While working to solve an important challenge facing patients with diabetes caused by autoimmunity, CRI Lloyd J. Old STAR Andrea Schietinger, Ph.D., of Memorial Sloan Kettering Cancer Center made a discovery with significant implications for cancer immunotherapy.
In type 1 diabetes, a person’s immune system attacks normal insulin-producing cells in the pancreas. During her research, Dr. Schietinger identified a population of autoreactive stem-like T cell capable of replenishing themselves constantly. These stem-like T cells do not become exhausted over time despite their persistent activity. This contrasts with T cells that lose their potency when fighting cancer. Dr. Schietinger is now studying these specialized T cells in hope of learning how to confer their durability onto cancer-fighting T cells while also searching for ways to prevent their harmful effect in diabetes patients.

Advancing New Technologies to Accelerate Research and Clinical Innovation
Technological innovation is a fundamental component of scientific research. New technologies make possible new tools that facilitate discovery and enable breakthroughs. As a result of technological progress, the rate of innovation in cancer immunotherapy research is accelerating.
Advances in genetic sequencing, computing power, artificial intelligence, imaging, laboratory research modeling, and more have pushed the fields of immunology and tumor immunology forward at breakneck speeds. These advances make it possible to ask questions about—and find solutions to—scientific puzzles that simply could not be explored before due to cost, time, and other factors that slow the pace of progress.
CRI scientists not only harness cutting-edge technologies to carry out their research, but they also are inventing new technologies needed to make the next great leaps in cancer immunotherapy.
Discovering New Targets for Cancer Immunotherapy
The growth in the number of available cancer immunotherapies over the past decade reflects the field’s excitement for the potential for these new treatments to help more patients control and possibly be cured of cancer. While there are now more than nine checkpoint blockade treatments and treatment combinations approved for more than a dozen major types of cancer, together these treatments target just three pathways involved in T cell suppression—CTLA-4, PD-1/L1, and, most recently, LAG3, which earlier this year became the first clinically effective novel checkpoint target to be FDA approved in almost a decade.
CRI scientists are seeking to increase the number of treatment options available to patients by identifying new targets of immunotherapeutic intervention. Given the complexity of the interaction between cancer and the immune system, many potential targets exist. While some have been characterized previously, few have resulted in meaningful changes in patient outcomes so far. New technologies developed by CRI scientists, however, are accelerating the search for targets with the greatest potential to improve patient responses to immunotherapy.
CRI Lloyd J. Old STAR Alexander Marson, M.D., Ph.D., at the University of California, San Francisco, and the Gladstone-UCSF Institute of Genomic Immunology developed a new CRISPR-based platform, hailed as a “Rosetta Stone” that has unlocked a new way to test any gene’s function in the human genome. This approach, called CRISPRa, allows Dr. Marson to rapidly find genes that can be “turned on” to enhance immune cell function. Using this technology, scientists can engineer T cells with very precise properties—which will facilitate the development of next-generation CAR T and other T cell-based immunotherapies. Dr. Marson, in collaboration with his colleague and fellow CRI Lloyd J. Old STAR Ansuman Satpathy, M.D., Ph.D., at Stanford University and Gladstone UCSF, conducted the first-ever genome-wide screen of genes involved in T cell exhaustion. Using a gene knockout approach, they were able to identify master genetic switches involved in this process. Manipulating these master switches may yield new insights leading to the prevention or reversal of T cell exhaustion to improve cancer patient responses to immunotherapy.
Tracking T Cell Movement with Greater Precision
These discoveries only scratch the surface of the factors that regulate the relationship between tumors, T cells, and myeloid cells that ultimately determines immunotherapy’s effectiveness and patient outcomes. In addition to how these cells act, where they act is also important.
To improve our ability to gain insights from clinical trials, Heike Daldrup-Link, M.D., Ph.D., a CRI Technology Impact Award Investigator at Stanford Medicine, developed a process using an FDA-approved iron oxide nanoparticle that enables visualization of CAR T cells within their patients. This new imaging approach enables doctors see where these cells move within the body and whether they are penetrating cancer’s defenses. This technology also enables tracking CAR T cell viability over time, information which may guide future therapeutic strategies.
It is important not only to know which types of immune cells have infiltrated the tumor microenvironment or what their functional status is (i.e., are they active, exhausted, anti-tumor, pro-tumor, etc.), but also to understand where these cells are in relation to one another and how this structural organization drives disease progression or response to therapy.
Brian Brown, Ph.D., a CRI Tech Impact Award Investigator at the Icahn School of Medicine at Mount Sinai, developed a functional genomics approach called Perturb-map, which renders three-dimensional visualizations at the single cell-level within tumors. Using this approach, Dr. Brown was able to define the tumor landscape with respect to the distribution of tumor growth factor-beta (TGF-β). This pathway is often associated with metastasis and poor patient prognoses, and Dr. Brown demonstrated how its signaling acts on the myeloid cells that then remodel the tumor microenvironment to physically exclude T cells.
Chip-Based Models of Host-Microbiome Interactions
The latest science is clear: the bacteria living within our intestines affect patient immune responses to treatment with immunotherapy. Just how this occurs is the subject of intense investigation as CRI scientists delve into the molecular mechanisms of host-microbiome interaction. Their goal is to identify new biome-based strategies to enhance patient immune responses to treatment. CRI Technology Impact Award Investigator Hyun Jung Kim, Ph.D., at The University of Texas at Austin has developed a novel “gut-on-a-chip” system that will accelerate research in this area. Dr. Kim’s three-dimensional chip mimics the complexity of the human intestinal microarchitecture, and allows him to model various interactions between the immune system and the microbiome. Ultimately, unraveling these connections may yield new insights into how such interactions contribute to or prevent the development of colorectal cancer as well as identify promising targets for new immunotherapy approaches.

Overcoming Immunosuppression in the Tumor Microenvironment

It has taken longer, however, to understand how other cells of the immune system can be enlisted to enhance patient responses. CRI scientists are working to discover how to do just that by looking at immune cells other than T cells.
Defending Against the Treachery of Macrophages
Myeloid cells are a group of immune cells that originate and mature in the bone marrow. These cells are major actors in mediating patient immune responses and cancer immunotherapy treatment outcomes. This stems from their ability to either promote or inhibit T cells responses. This family of cells includes macrophages, dendritic cells, granulocytes, and monocytes, among others, and they can contribute to the aggressiveness of cancer growth and spread throughout the body.
Macrophages are particularly important to the process of immune response mediation. Like generals, they dictate the course of the engagement and often determine whether the immune system and cancer-killing T cells will be suppressed or emerge victorious. Tumors, however, have an unsettling ability to convert a patient’s macrophages from defenders to turncoats, recruiting them in aiding cancer instead of fighting it.
How tumors do this is a key question in cancer immunology. Discovering the molecular mechanisms that give tumors the power to “turn” macrophages may provide valuable targets for new immunotherapies, unlocking new treatment strategies.
Ping-Chih Ho, Ph.D., a CRI Lloyd J. Old STAR at Ludwig Cancer Research and the University of Lausanne in Switzerland, recently revealed one way that tumors manipulate these macrophages. By producing molecules called glucosylceramides, tumors can disrupt the composition of macrophage membranes and induce internal cellular stress that results in immune suppression. This behavior, Dr. Ho discovered, depended on the presence of a transcription factor involved in inflammation, XBP1, which activates genes within the macrophage that transform them into immuno-suppressive, pro-tumor agents. Targeting this factor therapeutically is one potential approach to preventing this kind of cellular betrayal.
Myeloid cells also play a prominent role in immunotherapy-resistant pancreatic cancers. Here, the build-up of damaging reactive nitrogen species (RNS) acts as a barrier that keeps T cells at bay. Vincenzo Bronte, M.D., a CRI CLIP Investigator at the University of Verona in Italy, explored how we might reprogram myeloid cells and reverse the situation. In mice, Dr. Bronte demonstrated that an FDA-approved antiviral, remdesivir, blocks enzymes that produce the nitrogen compounds, making a type of immunotherapy called adoptive T cell transfer more successful. This work will be advanced through preclinical and potentially clinical studies to test whether this approach may be an effective way to improve the responses of patients receiving adoptive T cell therapy.
Harnessing Innate Immune Cells
Unlike myeloid cells, which originate and mature in bone marrow, another lineage of immune cells called lymphoid immune cells, while also having their origins in bone marrow, must travel to lymph nodes to mature. Gregory F. Sonnenberg, Ph.D., a CRI Lloyd J. Old STAR at Weill Cornell Medicine, led research that revealed an important link between colorectal cancer and immune cells called group 3 innate lymphoid cells (ILC3s). ILC3s patrol our intestines and, when functioning normally, ignore “good” bacteria in our gut and attack “bad” bacteria by initiating an inflammatory response that alerts other immune cells to danger.
Dr. Sonnenberg observed, however, that the number of ILC3s in colorectal cancer patients is significantly low and their function dysregulated compared to healthy individuals. Improperly functioning ILC3s can produce excessive inflammation in the intestines that, over time, can result in colorectal cancer. Their dysfunction may also result in their inability to control harmful bacteria in the gut, which in turn affects the immune response to cancer. Patients with few ILC3s also tended to respond poorly to treatment with checkpoint blockade immunotherapy.
This discovery may lead not only to a way to identify people at risk of developing colorectal cancer, but also may yield strategies to prevent or treat this type of cancer that is estimated to kill more than 52,000 U.S. residents each year.

Innovating Clinical Trial Design to Speed Discovery and Accelerate Cures

The CRI Anna-Maria Kellen Clinical Accelerator is a unique model for conducting early-phase clinical studies of novel immunotherapy combinations. Guided by the CRI Network of expert physician-scientists with input from collaborators at other nonprofits, academic institutions, and pharmaceutical and biotech companies, CRI clinical trials are driven by the latest scientific data as well as the greatest opportunities for saving more lives, including finding more effective ways to help people with especially hard-to-treat cancers. CRI trials are built upon solid scientific rationale and are designed not only to help patients now but also to yield new insights that will benefit more patients with other cancers now and in the future.
Over the past year, CRI made headway in several hard-to-treat cancers, including pancreatic cancer—one of the deadliest cancers given the late stage at which it is typically diagnosed and the ineffectiveness of existing treatments. As part of the immunological monitoring done on patients who participated in CRI’s PRINCE pancreatic cancer immunotherapy study, CRI scientists have identified in patient blood samples what they believe may be a predictive biomarker of responses to treatment with immunotherapy. CRI has funded a follow-on study with Robert H. Vonderheide, Ph.D., D.Phil., at the University of Pennsylvania, to evaluate the strength of this potential predictor. If confirmed, CRI plans to open a cohort in its REVOLUTION platform trial for patients with this biomarker.
In another clinical study, AMADEUS, CRI researchers found that a combination of immunotherapies was able to drive T cell infiltration into different types of tumors that previously had few infiltrating T lymphocytes. With enrollment complete of ninety-seven patients in the first two trial cohorts, interim data reveals a possible predictive biomarker in these patients as well.
CRI opened a new trial testing a combination of immunotherapy with two other drugs in patients with advanced stages of an aggressive subtype of ovarian cancer that no longer responds to treatment with platinum-based chemotherapy. This platform trial will allow CRI scientists to test the combinations and, based on what they learn from patient responses, modify the study design as needed to generate the best possible patient outcomes without having to initiate a completely new trial, saving time and accelerating clinical discovery.
